Pay attention to direct file changes, update post and notify user

Description

When opening the code editor for a file which changed since the last edit the user should be notified with a link to view the changes in the revision viewer.

a) Check for direct file changes
b) Update post
c) Display admin notice

In 2102:

Notify user when file changed since last editor visit. See #303
This uses md5 checksums instead of file last modified timestamps because we can generate the md5 before the changes are actually written to the file. This is important because atm no core changes are applied.

On a direct file change of a file edited in the wp editors before..

  • Notify user (in [2102] and [2103])
  • Update post in order to add a new revision
  • Provide user with a link to directly view changes
